An essential part of Project Open’s mission is to provide Lincoln Towers senior residents with the services and support they need so that they can remain in their apartments independently and “age in place.” To achieve this goal, Project Open provides both direct services to residents and helps arrange for services from other senior-serving agencies. Our Social Worker meets with individuals to assess their needs and create an Action Plan. The Plan is monitored and modified as an individual’s needs and abilities change over time.
